A lot of respons, yet an addition of mine.

Sorry, forgot the resitor between the output pin and the base of the transistor. This to keep the current low. 10k will do.

The voltage off the output will not be a bit higher then 0v when the port is off. To be sure that the transistor doesn't conduct you can place a resistor of 10k between base and emitter.

The free wheel diode is indeed beter placed over the relay.
