Well, sometimes you are in between... Or you don't have the money but expect to sell 1E6 (or whatever number of) units...
If you are planning to make 1.000.000 units and have no money GET A LOAN. You will never be able to make such a run without spending money up front. Try makeing a competetive product at volume using no custom parts (it will be unattractive) using no volume purcahsse (it will be expensive) without automatic insertion equipment (it will be labor intensive). so - you have no choice.
