Do you have some other means of programming than ISP? - I mean a parallel programmer, maybe just borrowing one to try if it makes a difference.
If it does, I would try programming at different clock speed and also set frequency=0 when programming - I have no non-H, but the H version uses a different ISP algorithm when setting frequency=0 and I think that is the proper one - uses the internal hardware "programming finished" flag; while if you enter a nonzero frequency, it times the programming using loops.
