If you are on the "other side" you might perhaps need some sleep. No, seriously. This sort of errors happens to me regularly when I try to work late in the night (sometimes literally falling asleep on the keyboard). Last night for example I discovered an error I made a day (night) earlier - I wrote a short cycle and forgot to change the cycle counter (so it looped in there forever).
